What if my child needs to take medication at school?
Superintendent Policy 5420 states that ALL medication taken at school, both prescription and over-the-counter, must be authorized by both the child's doctor and parent. Prescription medication needs to be in the original pharmacy-labeled bottle. Over-the-counter medication needs to be in the original packaging. All medications must be provided by the parent, and the dosing instructions on the package must match the authorization from the doctor. Please fill out and return the completed form to the office.
